Todoroki is the last name, remember Japan uses last names to address people unless they're really close. Iida and Midoriya are both last names too.

All might is a real hero cuz he prefer saving lives than going after fame and fortune, which is what the other heroes are doing. Deku was worthy and that's why stain didn't kill him.

I feel like the Dub didn't do as good a job there at the end as the Sub did. IIRC in the Sub they specifically say that they couldn't move because of the force of his willpower/belief. Something like that.

Stain believes all the heroes are "fake heroes". When he says All might is worthy he is basically just saying All might is the only real hero.
