She isn't reacting to the filler, she said that. It's just a question of if there will be a delay or not uploading the next bleach reaction. It sounded like she expected a delay when she recorded this, but a couple weeks ago she has said she finished the filler arc in her discord so maybe she's ahead of the expected schedule and bleach reactions will continue soon. Idk.
Soul Reapers of a high level are harder to kill than humans, they are spirits. This survivability scales even higher as you get to Captain level and beyond, it seems to be proportionate to spiritual pressure. Soul Reapers of that level can withstand a lot of punishment. That's why you see Kenpachi tanking all these hits, he has monstrous spiritual pressure. Even irl humans usually survive being stabbed (even to important organs) if they get fast medical attention believe it or not. It's kind of a hollywood myth that people just die right away when stabbed and can't be saved. To summarize, Momo was only stabbed once, was far more durable than a human, and was healed quickly by the best healer in Soul Society. Not to mention this is supernatural Soul Reaper magic healing, probably better than modern medical tech. So I think it makes sense she survived. With Bleach, you do have to get used to the fact that you can't be sure if a character is dead unless you are told they have died, or they have some insane injury like being decapitated or cut into pieces. They aren't invincible or anything though. Renji, Momo, and Toshiro all would have died from their wounds if Unohana had not treated them quickly. Getting treated quickly was the decisive factor.
As for Toshiro, I think it's clear that he feels guilty. He wasn't able to protect Momo, he made mistakes that allowed her to almost die. The two main things I can think of are that first he left Momo unguarded by any personnel to keep her from leaving, even knowing she was a target. He should have either stayed himself to watch her or had Rangiku stay. Someone like another Vice Captain that Momo couldn't easily defeat by force. He did put up his barrier, however, as we know the barrier failed because it is not designed to keep people in it's just an external barrier. That was quite careless considering Momo was not mentally stable, she left as soon as she woke up and started following him around to learn the truth. The 2nd mistake is that he didn't notice Momo's presence at all as she was following him for so long. It's possible Momo was so good at suppressing her spiritual pressure and being stealthy that he couldn't have noticed, but he will likely blame himself regardless. As a Captain, a Lieutenant should probably not be able to tail you for that long without you noticing. He might have lost focus from what had happened with Momo and that's why he didn't notice, it might have been his fault in that sense.
