When you get pierced by the Totsuka blade your soul is sucked into that gourd, and you're trapped in an eternal genjutsu of some kind. It's arguably a worse fate than death because your soul can't move to the afterlife.

Apparently there is a novel that tells the story between this arc and the next arc, which unveils that Itachi had some sort of desease which could did not have a cure at the time and thus his death. Even though they mention the whole backstory behind Itachi, they never mention about the desease in the anime. Novel name

People don't absorb other people when they seal them. You saw already the third hogake seal orochimarus arms. Whatever gets sealed, is sealed away, for example reaper death seal (tummy of the reaper) or here in a genjutsu world.
