Personally, I think the Author's implication by Nemu is that Uryu did, in fact, aim for Mayuri's body rather than his head. Uryu wouldn't miss someone's head when they didn't even have time to dodge.
Now, did he purposefully intend to have Mayuri live? I doubt it. If Mayuri had not liquified with his unique ability, he would have died FOR SURE from that massive wound regardless of any medical attention given. I find it likely that as Uryu was thinking of avenging his grandfather, he subconsciously spared Mayuri instant death since his Grandpa's dream was for Quincies and Soul Reapers to co-exist. I believe it's just meant to show that Uryu is not a character driven by revenge towards soul reapers, he shows a certain level of "kindness" even to his greatest enemies.

Also, Tosen (the guy who knocked out Ishida) is a Captain, yes. An easy way to tell if a Soul Reaper is a Captain is if they wear the white Captain jacket over their normal black Soul Reaper robes. It signifies their status as Captains.
The design of the Captain's jackets have some subtle differences between each Captain to express a bit of individuality, but they are always white with black line patterns near the bottom. Kyoraku Shunsui (Captain of Squad 8, the guy who defeated Chad) wears a pink Kimono over his Captain jacket, but he still wears it underneath the Kimono.
