Details that anime doesn´t explain so weel but the light novel did:
-PoH hates all the japanese, because his fater was japanese, and his step brother too. In fact PoH recognizes that Kirito look a little bit similar to his step brother (but only because the two are japanese)
-Vecta/Subtilizer was injured with the Sinon´s attack because he ¨thinks¨ that the guns are the only thing that can hurt him, and remember Suzy, he is trying to capture Alice.
-Neither of the caracters who logged into the Underworld can really die because his HP drops to Zero, neither Sinon, Leafa or Asuna. Their pain still real but anyone except Underworld´s people really die die inside of the Underworld.
-Poor Leafa but i respect her in this episode, in the light novel she had a lot of more wounds that she had, with ten swords inside her body.
-Eiji and Yuna didin´t appear in the light novel because it was written in 2014, but the movie was created and released in 2016-2017, the two of them still be canon anyways

I mean, we already saw in part 2 of S1 that dmg in the virtual world can actually affect the real body, with the whole pain inhibitor and stuff.
The elf king guy or whatever had his eye clearly affected and nearly blinded after Kirito pierced his face with the sword in the virtual world.

BTW I'll add some info. There is an official short story that tell how Kirito and Eiji made amends after the movie ended. Kirito was having visions of Yuna still, even after the movie. And Kirito and Eiji worked together to help what was left of Yuna. (I could be wrong on points, long time)

This is of course an amazing episode but remember that just like the 2 american mercs that are in the underworld when they first dived in, Asuna, Leafa and Sinon are not using their own personal accounts. They are using the admin accounts so even if they are killed in the underworld, their accounts will be safe unless they need to use them to dive back in a second time.

First Off in the light novel vassago was a bit rougher with kirito He was really pissed off that after a two year wait Kirito was broken and he didn't break him. he wanted to kill kirito fair and square thats why he was trying so hard to wake him up even threatening to kill every fluctlight in the cluster.
second The Eji and Vassago fight was anime original it was not in the light novels just like the movie.
thirdly Leafa's Goddess acount terraria gives her unlimited health and regeniration so she cant die
Finally The three people that Kirito is closest two in the real world are Asuna the Love Of His Life His Sister and Shino who is like a bestie to him. Keep that in mind for Kirito's nightmare sequence in the next episode Unfortunately A1 Pictures didnt adapt it that well so brace yourself for the next episode

There's a lot of difference between PoH's background in the anime and the Light Novel and I feel that I just need to mention it. His mother named him "Vassago" which is a demon that is known as "The Prince Of Hell". His mother was a prostitute that accidentally became pregnant with his father. However, the father made her keep him as his older son had a kidney illness and would eventually need a kidney transplant. When his father told him he'd have to be a kidney donor for his brother, he made a deal that in exchange he could go to Japan to hopefully start a new life there. However, due to Japan's laws, he was illegal on arrival, and was forced to become a hitman for a Korean Crime Syndicate where he was eventually forced to go kill someone by diving into the SAO death game and he had no choice. Although when he was in SAO he "changed". He felt more free than he ever did. All the Japanese in SAO reminded him of his father. So he set out to kill his original target and as many other japanese as he could.
